Certificates of Insurance
A certificate of insurance is a document used to verify that an entity is insured. In order to request evidence of CSU’s insurance, complete the Certificate of Insurance Request Form. Make sure that you include a copy of the agreement. If the insurance language is correct, and if other terms of the agreement have been approved, then CSU’s certificate of insurance will be issued.
Once completed, email the Certificate Request Form and a copy of the agreement to Joyce Pratt.
